To make an appointment, please call our office and one of our staff members will answer all of your questions and assist you in your request.
We will gather information about your sleep concerns to schedule the right type of appointment for you.
Although a referral may not be necessary, please let us know if you have one so we can forward the results to your referring physician.
We will mail you a “Welcome Packet” with information about your appointment, registration forms, and our privacy policy.
Please make sure you read the instructions carefully.
Please complete and bring any forms enclosed in the "Welcome Packet."
Please bring your photo ID and your insurance cards.
Please arrive a few minutes early to complete the registration process. Any co-pays are due at the time of the visit.
Your time is valuable, and we make every effort to keep times in the waiting room to a minimum. Unfortunately, on occasion we get backed up, and we appreciate your patience as we are always trying to ensure all of our patients are well attended.
Office visits durations may vary according to the nature and complexity of the problem.
If you need to change or cancel your appointment, please call the office three business days before your scheduled appointment. Patients who do not show up for their appointments are subject to a missed appointment fee.
